Assuming the midline location is important it could be a thyroglossal duct cyst. Any inflammation and discharge mean you have to exclude infective conditions including TB. I would examine the discharge microscopically and culture for TB, Leishmania etc. A desmoid cyst can occur here. A branchial arch cyst would be rare.
